pilot lost #2 engine and then throttled back #3 accidentally, flying on "2" engines........

at takeoff they got an indication(later found to be erroneous) of a malfunction in #2 engine. so they followed procedure, circled above dover, shut it down. then when making the approach for landing he left the #3 throttle down and advanced #2 which was off, therefore only flying with 2 engines at power, one at idle.

that plane had just finished a multi-million dollar to upgrade the avionics, install 'glass' cockpit. and those were the reserves flying. the unfamiliarity with the new style of engine parameters displayed on the glass cockpit they never noticed they had the wrong engine pushed up.
